The attached test file fails to render properly in Ghostscript, but works fine
in Acrobat Reader 5.  All the glyphs show up as empty boxes in Ghostscript and
there are numerous messages like:

Substituting .notdef for V
Substituting .notdef for B
Substituting .notdef for P
Substituting .notdef for F
Substituting .notdef for parenleft

Fixed with a (hopefully rarely triggered) heuristic improvement that
uses the format 3,0 cmap when the 'post' doesn't contain glyphs that
are in the 'prebuilt_encoding' derived from the /Differences info in
the PDF Font /Encoding info.
