I have quickly added a sentence about "-dDisableFAPI=true" in r11228 (and see also
http://bugs.ghostscript.com/show_bug.cgi?id=691290#c5) but I think some of the current documentation is out-dated and possibly wrong, such as the part in 
"Appendix: Running Ghostscript with third-party font renderers" about pdfwrite.

I think the documentation is now sufficient for this.
