When read this pdf by mupdf.exe(2009-12-05), it failed. But acrobat reader and 
xpdf are correct.

The first "error when read pdf" problem has been fixed by adding the UTF16 CMaps:

The fonts that should be bold but aren't problem remains.

The bold text is created by drawing text with stroked render mode.
