Customer reports and I've verified that neither Ghostscript 8.57 nor gshead
(r8270) can't read the attached PDF file.  

Ghostscript generates a status message followedby an error:

  Substituting CID font resource/Adobe-Identity for /CourierNew.
  Error: /undefinedresource in findresource


The command line I'm using for testing:

  bin/gs -sDEVICE=ppmraw -sOutputFile=test.ppm ./bad.pdf

Evince and Acrobat Reader 8.0 open the file without problem.
